Statement of Problem: Community-based health service providers serve an integral function in health programs due to their being embedded in the communities. Community Health Workers (CHWs) are a range of workers who receive less formal training than professional counterparts. Community Mental Health Workers (CMHWs) are a type of CHW with distinct differences in the burden and responsibilities of their work, as well as unique sets of challenges and tasks facing CMHWs on the individual. CMHWs are being employed using task shifting, which is the inclusion of non-technical workers in jobs that would otherwise have to be completed by highly skilled health workers. Current literature points to generalized reasons for motivation and retention, while noting the importance of context-specific factors, however, as a relatively new cadre of workers, data originating directly from CMHWs regarding reasons for motivation and retention is lacking.

Methods: This study was conducted under a parent project randomized controlled trial that started in 2010. Participants for this study included counselors (n=20) and clinical supervisors (n=3) working on the parent project through their respective organizations. Four rounds of in-depth interviews were conducted to collect data in order to develop a theory of CMHW motivation.

Results: Although individual-level factors play a large role in motivation, most important are issues related to system functioning. Inadequate pay, lack of job security, and poor communication were reported to greatly affect CMHW motivation. CMHWs were able to overcome most obstacles they faced in terms of community- and client-level factors; however, the financial systems of their organizations and a lack of continued funding led to changes in motivation, increased potential for attrition, and anxiety for the CMHWs.

Conclusions: The results indicate that efforts to ensure client and community support for the work of CMHWs, as well as inputs such as training and supervision, can contribute greatly to service provider motivation. In addition, long-term financial stability of the organizations employing these service providers is essential for continued motivation and retention.
