The main goal of this study was to define and describe problems in marital relationships as perceived by infertile women living in northern Taiwan. The primary purpose for undertaking this study was to improve nursing care, especially counseling, for infertile Taiwanese women. Specifically, the aims were to: (a) identify the essential elements of infertility; (b) identify the essential elements of marital relationships within the context of infertility; and (c) describe the experiences of infertile women who have dealt with issues based on infertility with their spouses. Fifteen subjects were interviewed twice. Initial conversations were audiotaped and transcribed; transcripts were used to identify potential themes and to verify certain points made by each subject. Major themes identified were a sense of failure, I chu chih han ("Regret over the loss of a pearl."), pursuit of a dream, and enduring a waiting process. Relationship themes were identified as a connecting process, T'ung chou kung chi ("We are in the same boat"), making efforts to get pregnant, maintaining harmony, feeling a lack of connection, taking risks, and refocusing/relocating a marriage. Examined within the context of Taiwanese culture, these themes were found to be directly connected to three underlying cultural aspects of infertility: power, face, and fate.

The author offers suggestions on how health professionals can better provide a protective environment for infertile couples---one which ensures face-saving privacy for patients undergoing treatment or counseling. Specifically, education is recommended for the purpose of improving communication between nurses and reluctant patients on the issue of infertility. Recommendations are made for encouraging Taiwanese husbands to accompany their wives for clinic appointments, and a model for community- and computer-based support groups is discussed.
