The continuing increases in the cost of healthcare have forced the medical community and third-party payers to examine ways to hold the line on medical costs without reducing the amount of care provided. One of the continuing efforts in this regard is the use of case management (CM). Long a factor in controlling costs for workers' compensation and chronic or acute medical and surgical care regimens, case management is now being used for maternity and neo-natal care.Subjects--Topical Terms:

Case management is currently being used in the care of both the insured and uninsured. Most previous studies have focused on the value of CM in caring for the poor, minorities and high-risk patients. The focus of this study is the value of CM in treating middle-class, insured patients, from the perspective of both cost and quality of care. The data comes from a population of approximately 10,000 employees and their families. During the year of the study, there were 200 completed pregnancies and data on these pregnancies is the data for this study.

This study uses various univariate and multivariate tests to determine whether case management influenced the hospital costs of pre-natal, delivery and post-natal care, as well as the value of CM in two quality factors: birth weight and APGAR scores. The analysis includes investigation of the relationships of variables including mother's age, attending physician specialty, employee/dependent status, point in pregnancy for first medical visit, and when certified in case management.

The results indicate that there are significant differences in the cost of pre-natal and post-natal care. No significant differences were found in hospital costs or quality, outcomes very different from studies using disadvantaged groups.
