With Kenneth Chay and Michael Greenstone, chapter one seeks to explain the stunning decrease in infant mortality among African-American children which occurred during the 1960s. The 1965--1971 reduction accounts for the greatest convergence in black-white infant mortality rates in the entire post-World War II era. These mortality improvements were concentrated in the southern United States, where racial segregation in hospital services was both legal and pervasive through 1964. Moreover, the 1960s mortality reduction was largest for causes of death considered preventable by medical treatment, such as pneumonia and gastroenteritis. Analyzing a unique county-level dataset from Mississippi, we conclude that the integration of hospitals, spurred by the financial incentive of hospitals to participate in the new Medicare program, played a causal role in reducing black mortality.

Chapter two investigates the extent to which health as an adult is affected by one's health as a child. Between 1918 and 1937, overall infant health conditions improved tremendously in the United States. The timing and geographic location of these improvements help predict the substantial improvements in adult health observed over the last thirty years. Focussing on the sudden deterioration of fetal and infant health during the 1918 influenza pandemic, the likelihood of death during the 1980s is seen to be affected by differential in-utero exposure to the 1918 pandemic, implying that early-life health conditions exert a large effect on an individual's health throughout life. Establishing a link between early childhood and adult health has immediate implications for both health and public policy. As health expenditures are currently concentrated in the last years of life, reallocating funds to early childhood periods would be the most efficient way to improve adult health and longevity.
