This study examined the data from an ongoing study, "Impact on Child Development of Early Otitis Media", to determine the association between Otitis Media with Effusion (OME), during the first three years of life, the relative hearing loss, and the subsequent cognitive and language acquisition outcomes, for children at age five. A structural equation modeling approach was used to estimate theoretically relevant interrelations among OME, hearing loss and these outcomes.

This study extended the research on the effects of OME on the development of children by incorporating the change parameters (intercept, slope, and curvature), based on OME measures between the ages of six and thirty-six months, and hearing loss measures between the ages of twenty-four and sixty months, as constructs. These constructs were then used to predict the outcome variables, intelligence and language acquisition, at age five, with SES and mother's intelligence as control variables.

Data previously collected from 300 normal children observed from birth to age five was used to address the research questions. The statistical analysis was performed with the software program LISREL, a linear structural equation modeling procedure that uses maximal likelihood estimates to examine interrelations among variables.

The results of the Structural Model for language indicated that OME was associated with hearing loss in children, and that hearing loss was related to the language performance of young children. The effect of OME on the language scores at age five was indirect and mediated by hearing loss, while OME and hearing loss were not related to the intelligence scores of the children at age five.

The results therefore extended the research on the effects of OME on the development of children by looking at change from a different perspective, one in which the change parameter constructs were linked to the outcome variables. Based on these results, it is suggested that pediatricians treat OME in children aggressively, and that educators develop remedial programs for children who have had incidences of OME infections and are entering the school system, in order to help them minimize deficits in language caused by the effects of OME.
