The study purpose was to investigate the association between periodontal status and cardiovascular disease (CVD) by prospective and cross-sectional analyses of national data.

The prospective analysis used data from the First National Health and Nutrition Examination Survey (NHANES I) and Epidemiological Follow-up Study. 9,962 people free from myocardial infarction, stroke, heart failure and cancer at baseline were followed for an average of 21 years. Baseline periodontal status was categorized into (1) no periodontal disease, (2) gingivitis, (3) periodontitis, and (4) edentulousness. Non-fatal events were ascertained by hospital records and fatal events by death certificates. Life table analyses and Cox regressions were performed.

The cross-sectional analysis used data from NHANES III. Periodontal assessments included gingival bleeding, calculus, pocket depth, and attachment loss. Dependent variables were total and HDL cholesterol, C-reactive protein, and fibrinogen. The study sample sizes were 10,146 for cholesterol and C-reactive protein, and 4,461 for fibrinogen. Analysis of covariance and multiple linear regression were performed.

The prospective analysis found that periodontitis was associated with increased risks for incident CVD, coronary heart disease (CHD), and stroke. The association was consistent for men and women, and for Non-blacks and Blacks, while the association with CHD in women and in Blacks did not reach significance. On average, compared to those with no periodontal disease, people with periodontitis appeared to have a 25% increase in incident CVD risk, a 20% increase in incident CHD risk, and a 66% increase in incident stroke risk. The relation between periodontal disease and the fatal events was similar to that found for the incident events. Overall, periodontal disease might account in the population for 6% of CVD incidence, 13% of stroke incidence, 5% of CVD mortality, and 21% of stroke mortality.

The cross-sectional analysis found a significant relation between periodontal status and increased C-reactive protein and fibrinogen. The association with total cholesterol was weaker. The relationships with these CVD risk factors were generally consistent for Whites, Non-Hispanic Blacks, and Mexican Americans. These relationships with CVD risk factors may explain, at least in part, the association between periodontal disease and increased CVD risks observed in the prospective analysis.
