Exclusive and almost exclusive breast-feeding (i.e., full breast-feeding or FBF) in the first six months of life are important contributors to infant health worldwide, yet the duration of these practices remains low in many parts of the world. Perceived insufficient milk (PIM) is among most common reasons cited by women for the early supplementation and/or discontinuation of breast-feeding and has been related to maternal concerns regarding infant crying behavior.

The aim of this thesis is to comprehensively explore maternal conceptualization of PIM, document maternally-reported infant crying patterns and examine the associations between crying patterns, insufficient milk and infant feeding decisions.

To accomplish this we conducted a two-phase study, composed of an initial formative qualitative research period followed by an observational cohort study, in which mother-infant dyads were recruited at birth and visited in their homes 9 times from 1 week to 24 weeks of age.

Our findings included the estimation of a two-fold increased risk associated with the abandonment of FBF among women reporting PIM. We also confirmed the association between perceived insufficient milk and reported crying patterns and provided further detail as to the nature and the estimated magnitude of the association. Maternal self-efficacy measured in the post-partum period was also found to affect maternal perceptions of crying behaviors, with their own effect on reported crying patterns and infant feeding practices.

We conclude that maternal report of PIM is an important factor in the promotion of FBF. Our findings also indicated that reported and perceived infant crying patterns as well as maternal self-efficacy have additional roles in this regard, both related to PIM and as independent factors. These findings and their implications are further discussed.
