Carpal Tunnel Syndrome is a common work-related musculoskeletal disorder. Aim one: to determine the effect of either voluntary low (LR/NF) or high (HR/NF) repetition tasks with negligible force on rat peripheral nerve by examining the production of inflammatory mediators IL-1alpha, IL-1beta, TNF-alpha, IL-6 and IL-10 using ELISA techniques. Each of these cytokines was increased in the nerves studied (median and sciatic). The median nerves of reach limbs and sciatic nerves of weeks 6/8 LR/NF rats contained significantly less IL-1alpha, TNF-alpha and IL-6 (median nerve only) than they did at 3 weeks, indicating a decrease in the inflammatory response over time. Median nerves of reach and non-reach limbs of week 9 HR/NF rats showed suppressed inflammatory responses (decreased IL-1alpha in both limbs and decreased IL-10 in reach limb). In contrast, the inflammatory cytokine response continued unabated in the median nerve of the partial reach limb and sciatic nerves of these same rats. Aim two: to determine the effect of acute nerve stretch on nerve conduction velocity (NCV) for two hours immediately after an acute stretch of 20% of nerve length. Conduction was slowed (n = 7) or blocked (n = 2) in two nerves immediately after stretch. During the first hour NCV increased then decreased again by the end of the second hour. The decrease in NCV over the second hour in both stretched and control nerves suggest either a halothane anesthesia effect or inadequate nerve perfusion to the nerve. Aim three: to determine the effect of high repetition/high force (HR/HF) task on duration of task participation, limb withdrawal threshold, and median nerve NCV. Eight rats were trained to perform a HR/NF reaching and pulling task for 12 weeks. Duration dropped by week 4 then fluctuated. The withdrawal threshold was significantly increased by week 12 in all 4 limbs when compared to week 0. NCV was significantly decreased by week 12 in both the reach and non-reach limbs compared to controls. The results indicate that repetitive forelimb movement damages tissues, alters behavior, and reduces physiological function of the median nerve.
