This dissertation will consider the different possible approaches towards the ontological question of the nature of diseases. Under this heading, it will pursue five goals. First, it will consider the ontological positions currently on offer in the domain of medicine. Second, it will survey the literature to see which of these positions various authors hold. Third, it will critique these positions, using both arguments raised in the literature (which will themselves be critiqued) and novel arguments. Fourth, it will consider whether a "best version" of each of the various types of account (specifically realist and anti-realist) can be proposed. Finally, it will describe a novel approach to medical ontology.

The dissertation begins in chapter 2 by considering the most frequently discussed question in this vicinity, the question of the distinction between disease and health, and suggests a new way of understanding the dialectic, usually portrayed, roughly, as a dispute between naturalists and normativists. This new taxonomy will reveal that it is no longer clear that there is a single question of "what is a disease" about which all authors are writing, but rather, multiple questions. The purpose of this section is to show why it is not necessary to answer the question "what is a disease?" before considering the nature of individual diseases.

Chapter 3 moves on to the nature of particular diseases, which shall be referred to, in order to avoid the question of the last paragraph, "conditions." Chapters 4 and 5 then discuss realism and anti-realism, respectively, according to the strategy outlined above. Both realism and anti-realism, we shall see, have significant weaknesses. These discussions of realism and anti-realism will be followed in chapter 6 by considerations of three different theories which each, in their own way, combine features of realism and anti-realism. These accounts too will be seen to be highly problematic.

Finally, chapter 7 presents a novel account of the ontology of conditions based on Giere's constructive realism which avoids the problems of both realism and anti-realism while retaining much of their appeal.
