Sudden cardiac death, caused by a life-threatening arrhythmia, can be forestalled with an implantable cardioverter defibrillator (ICD). The ICD will deliver a shock to a person's heart if a lethal ventricular arrhythmia occurs, facilitating the return of a normal heart rhythm. However, with this technology comes psychosocial issues, especially anxiety. Social support has the ability to influence anxiety levels and has been shown to have a positive impact on the health of cardiac patients. Support groups are one way to provide social support and expand the social network of people who have an ICD; yet very few studies have been done to assess the benefits of these groups. The purpose of this study was to determine whether participation in a support group influenced perception of social support and level of anxiety in patients who have received an ICD. In addition, this study identified the relationships among time, spatial location (urban or rural), social support, social networks and anxiety.

Study participants were 150 adults who did and did not attend an ICD support group. They were recruited from one hospital and 10 ICD support groups in New York, Pennsylvania and Connecticut. Data was collected using voluntary, anonymous, structured "paper and pencil" questionnaires. Questionnaires used were a demographic data form, the Sarason Social Support Questionnaire (Sarason, 1987), the Speilberger State-Trait Anxiety Inventory (Speilberger, 1970), and an ICD Support Group Questionnaire (Myers, 2003).

Data was analyzed using descriptive statistics, chi-square, Mann Whitney U, t-tests, two-way ANOVA, and Pearson correlation coefficients. Multiple regression analysis was conducted to determine the combined effects of the variables. It was determined that as satisfaction with social support increased, both state (r = -442, p = .000) and trait (r = -507, p = .000) anxiety levels decreased.

In addition, those with a larger social network had higher satisfaction with social support (r = .186, p = .014). Multiple regression determined that those who do not attend a support group, have more people in their social network and who live in an urban area will have greater satisfaction with support.
