The profession of occupational therapy faces challenges and opportunities brought about by changes in the health care system and other external demands. The context for service provision is expanding beyond facility-based environments, such as rehabilitation programs and hospitals, to homes and a variety of community settings. A vital component of this evolving future vision for the profession is the ongoing development of emerging practice.

The purpose of this exploratory study was to identify and investigate the nature of emerging practice within the profession of occupational therapy, its rewards and challenges, and the professional competencies for practice. For this study, emerging practice was defined as the development and provision of occupational therapy services in environments where services have not traditionally been available or well established. The study is important to the educational preparation and professional development of occupational therapists for current and future emerging practice roles and challenges.

Two phases of data collection were completed: (1) a Preliminary Survey, and (2) the Delphi method of forecasting and exploration. The purpose of the Preliminary Survey was to gather general information and opinions about emerging practice in the profession. The written Preliminary Survey was completed by 174 occupational therapists. The Delphi method was employed with an expert panel of 24 occupational therapists, currently practicing in emerging roles or settings, to gather specific information about the essential competencies and competency characteristics for emerging practice and to identify strategies for their future development. During the three round Delphi process, the panelists assigned a rank and weight to identified items according to a modified version of the Multi-Attribute Utility theory that led to a consensus about the relative importance of each competency, competency characteristic, or strategy.

Conclusions drawn from the Preliminary Survey data indicated occupational therapists hold diverse perceptions about emerging practice settings and services as well as the role and significance of emerging practice within the profession. Occupational therapists engaged in emerging practice described multiple rewards and challenges inherent within the process of developing and delivering services. The Delphi panel identified 104 competencies essential to emerging practice that encompassed knowledge, interpersonal abilities, performance skills, critical reasoning, ethical reasoning, and the characteristics of the individual. The Delphi panel identified 55 strategies for the development of these competencies pertinent to occupational therapy educational programs and professional organizations. The descriptions of emerging practice and identified competencies suggested leadership, innovation, and entrepreneurship are essential elements to its success.
