Objective. The influence of family on adolescent mental health service utilization has not been fully explored. This study examined whether father's, mother's, and sibling's perceptions of burden mediate the relationship between adolescent symptom severity and adolescent mental health service utilization.

Method. Longitudinal data gathered from 397 white families (a target adolescent, father, mother, and one sibling) participating in the Iowa Youth and Families Project were used. Data included symptom severity, family burden, family socioeconomic status, parent's mental health status and treatment, and the outcome measure of adolescent mental health service use.

Results. 76 (19.1%) adolescents obtained mental health services. Final analyses revealed no supporting evidence that burden mediates the relationship between adolescent symptom severity and mental health service utilization. Father's burden was associated with female adolescent use of specialist mental health services. History of parental mental health problems and parental mental health treatment increased the likelihood of mental health service utilization. Mother's, father's, and sibling's perceptions of burden were not highly correlated with each other.

Conclusion. Results highlight the importance of assessing family burden from multiple informants. Risk factors such as parental mental health history and treatment are important determinants in adolescent mental health service use.
