The prediction of more than 50 million postmenopausal women residing in the United States and living longer emphasizes the importance for healthcare providers to understand how women choose available therapies (alternative, traditional, or combined) for managing menopause and uncomfortable menopausal symptomatology. The Modeling/Role Modeling Theory was selected as the underpinning for this descriptive, qualitative study as the model affords the foundation for understanding people and the role of the nurse in the provision of optimum care.

The purposive sample was comprised of 30 menopausal and perimenopausal women who had experienced, or were experiencing, menopause as a natural transition of life. Content analysis was utilized to describe factors that influence women to decline HRT, choose therapies other than HRT, or do nothing and to describe and categorize therapies selected for uncomfortable symptomatology.

Recorded interviews conducted by the researcher were transcribed verbatim and, using the phrases as the unit of measure, specific factor responses for declining HRT were coded and assigned to previously determined categories gleaned from the literature (fear, lack of knowledge, financial concerns, past experiences, and other) in a multi-celled code book map. Specified interventions utilized, if any, were assigned to predetermined code book map categories (nutritional; spiritual; psychological; drugs and biological; physical forces, devices or touch; miscellaneous, and folklore and "old wives tales"). The completed code book maps contained responses in each of the factor categories and all intervention categories except folklore and "old wives tales".

Further analysis of verbatim responses led to the emergence of various concepts which initiated a theme development that reflected relatedness of behaviors, experiences, and activities of menopausal women. Five core categories evolved that suggested the women were "making their way" as if on a passage or journey through the menopausal phase of fife. The evolved core categories included (a) understanding the menopausal journey, (b) managing menopausal symptoms during the journey, (c) connecting with others during the journey, (d) acknowledging of self during the journey, and (e) approaching the journey's end.
