Results of epidemiological studies have established that a variety of influences collectively termed "risk factors" predict the future development of coronary heart disease and other clinical atherosclerotic diseases. It is believed that these risk factors contribute to the development of atherosclerosis by modifying the expression of genes and/or proteins that influence the susceptibility versus resistance of the vascular endothelium to the development of atherosclerosis. Notably, proteins that enhance the bioavailability of nitric oxide (NO), a key vascular protective molecule produced by the endothelium, are considered anti-atherogenic whereas proteins associated with inflammation, vasoconstriction, inhibition of NO synthesis, oxidant production, and oxidative damage are considered atherogenic.

520 $a The purpose of the present study was to determine the relations between cardiovascular risk factors and the vascular endothelial cell protein expression (VECPE) of atherogenic and anti-atherogenic proteins that influence nitric oxide production, oxidative stress (i.e., oxidant production relative to antioxidant defenses), inflammation, and vascoconstrictor synthesis. Endothelial cells were obtained from forearm veins of adult men and women and VECPE was measured by quantitative immunofluorescence.
520 $a Cardiovascular risk factors including obesity, sedentary lifestyle, elevated diastolic blood pressure, insulin resistance, and elevated blood lipids were associated with increased VECPE of proteins that inhibit NO synthesis and increase oxidant production. These risk factors were associated with presumably compensatory increases in VECPE of antioxidant enzymes and an enzyme of NO synthesis. Emerging cardiovascular risk factors including elevated serum oxidized LDL and free fatty acids were associated with increased VECPE of an inflammatory transcription factor, an antioxidant defense, and a marker of oxidative damage. Importantly, of all the risk factors measured, obesity had the most profound independent influence on VECPE.
520 $a In summary, cardiovascular risk factors are associated with a pro-oxidant, vasoconstrictor, and inflammatory endothelial phenotype featuring a compensatory up-regulation of antioxidant enzymes and NO synthase. By characterizing the VECPE phenotype associated with cardiovascular risk factors, insight has been gained into the molecular mechanisms by which cardiovascular risk factors may contribute to the development of atherosclerosis.
