While safety and health concerns have been raised by various professional groups regarding creatine supplementation, no clinical study has reported any side effects from creatine supplementation other than weight gain. However, French scientists reported that creatine may cause muscle cramps, kidney problems, and may be cancerous.

The purpose of this study was to evaluate, via survey, the perceived benefits and risks associated with creatine supplementation by bodybuilders and power lifters who used creatine. Sixty-four athletes; fifteen competitive bodybuilders (23%), five competitive power lifters (8%), and forty-four non competitive bodybuilders (69%) participated in this study. Subjects completed an anonymous descriptive survey on the prevalence of creatine use, the perceived benefits, risks of using creatine, and sources of information regarding creatine supplementation. Respondents were required to check "all that apply" from a benefits and risks list. These lists were generated from current information in the literature and from common concerns stated by health professionals. An alpha level of .05 was used for determining statistical significance.

Results. Most creatine users used the cyclic method of creatine supplementation. Creatine users listed the following as perceived benefits: increased strength, weight gain, reduced fatigue, decreased body fat, other, and none. Perceived risks were ranked as follows: none (73%), dehydration (8%), muscle cramps (5%), diarrhea (5%), headache (5%), kidney damage (3%), gastrointestinal distress (2%). Professional athletes (26.8%), friends (22.3%), magazines (10.7%), and the Internet (8.9%) were the most common sources of creatine information compared to nutritionists (2.7%) and physician (0.9%). This study found that: (1) awareness and use of creatine is widespread among this population; (2) most used a cyclic method of creatine administration; and (3) although most of them appear to be following the dosage reported in the literature, others may consume greater amounts or were not aware of the amount taken.
