Workplace dermal hazard in the U.S. is largely un-recognized and under-evaluated even though indications of a significant threat to worker health exist. Recent research suggests that behavior plays a significant role in worker exposure. However, no specific methodology has been developed to examine behavioral determinants. Thus the goal of this research was to examine dermal exposure assessment methods, worker dermal exposure and protection, and behavioral determinants of exposure.

To investigate the performance of dermal exposure assessment methods, we compared externally-derived dermal exposure measurements to urinary biomarker levels of a common agricultural pesticide, chlorpyrifos, in workers. Data from five pesticide registrant studies that included exposure measurements based on both external measurements and biological monitoring were used to examine methods of assessment, routes and determinants of exposure and dose to the pesticide chlorpyrifos. Eighty workers across four job classes included mixer/loaders (M/L, n = 24), mixer/loader/applicators (M/L/A, n = 37), applicators (A, n = 9) and re-entry scouts (RS, n = 10). Doses were highly variable and differed by job class (p < 0.05) with median total (inhalation and dermal combined) exposure-derived absorbed doses (EDADtot) of 129, 88, 85 and 45 mug/application for A, M/L/A, M/L and RS, respectively. A simple linear regression of urinary 3,5,6-TCP against EDADtot indicated a positive association explaining 29% of the variability in the 3,5,6-TCP derived estimate of dose. The gap between external and internal exposure measures supports the further development and refinement of dermal exposure assessment methods or alternative measures to assess worker dermal exposure.

We attempted to address this need by developing a questionnaire to assess worker and manager knowledge, attitudes, and perceptions (KAP) as an underlying determinant of behavior related to workplace dermal hazards. Seven scales were developed to measure behavioral concepts including knowledge, information belief, behavior, behavior beliefs, training, self-efficacy, and overall belief. Nineteen industries across the Baltimore, MD and York, PA region participated in the field evaluation portion of the study including a total of 89 workers and 17 managers, with approximately 5 workers per facility. Workers and managers self-completed the KAP questionnaire. Cronbach's alpha reliability scores for worker KAP scales (n = 89) ranged from 0.87 for the Self-efficacy PPE Scale to 0.92 for the Overall Belief Scale with an Overall Scale alpha of 0.86, indicating high reliability. The KAP questionnaire had face validity as well as content validity with respect to PPE barriers, PPE availability and access, worker risk perceptions, and hazard information sources and availability.
