That case discussion conferences (scheduled educational meetings at which patient histories are presented for discussion by residents and clinical faculty) are part of essentially all anesthesiology training programs suggests that they are perceived as important, but the variety of forms and styles suggests little agreement about what this importance is. Preliminary inquiry suggested presentation of information, sharing of uncommon clinical experiences, quality assurance and peer review, social solidarity, and practice in clinical problem solving as potential purposes. The kinds of conferences which would best serve these various needs appeared quite different and perhaps contradictory.Subjects--Topical Terms:

This study presents data from structured interviews of case conference organizers, questionnaires circulated to conference participants, and observations of conferences in seven university-affiliated anesthesiology residency programs in the Northeast.

There was considerable variability among these groups in perceptions of the function of case conferences, with but few systematic differences. Members of departments concurred to some extent. Faculty appeared more aware than residents of social functions; organizers noted the distinction between learning problem solving and learning information; but the major impression was that relatively little thought had been given to function, even by organizers.

Several conflicts resulting from this ambiguous sense of purpose were noted. Combining quality assurance and educational conferences appeared to give residents substantial ambivalence about reporting and presenting cases. Exhibition of diverse approaches led to some disagreements about which alternatives were acceptable; moderators were sometimes inadequately prepared for these. While practice in problem solving is judged the optimal function for case conferences (with other functions accomplished elsewhere), nearly two-thirds of participants had the advantage of knowing the general nature of the problems from references circulated beforehand, and outcomes of specific patients were known to a third ahead of time. This was noted even in conferences where problem solving practice was a clear objective of the organizer, and the resulting hindsight bias is of serious concern.

Recommendations are presented, including making practice of clinical problem solving the focus of these conferences and, most urgently, considering function carefully in each department, establishing a consensus among participants, and designing the format to support those functions.
