Phthalates are known reproductive toxicants that are ubiquitous in the general population due to their widespread usage in consumer products. Massage therapists are a potential occupational group at risk for exposure as a result of their high usage of numerous skincare products that commonly contain phthalates. The goal of this quantitative research study was to gain a better understanding of the source and extent of phthalate exposure from products used in the worksetting among massage therapy students.

This cross-sectional study involved administering a questionnaire and obtaining pre- and post-shift (before and after class) urine as well as cross-shift air samples. Sixty-one massage therapy students from a large therapeutic massage school in the Philadelphia area were enrolled. There were a total of 44 females and 17 males recruited for this study. Subjects were in the age range of 19--49 years (mean 31 years). Diethyl phthalate (DEP) and dibutyl phthalate (DBP) were present in 100% of the air samples, and monoethyl phthalate (MEP) and monobutyl phthalate (MBP) were detectable in 98.4% of the pre-shift samples and 100% of the post-shift samples. Data analysis involved descriptive statistics, t-tests, Analysis of Variance (ANOVA), and generalized linear models for analysis.

The correlation between DBP and DEP air levels and log transformed creatinine-corrected urinary biomarkers MBP and MEP was statistically significant, P=0.04 and P=0.04, respectively. Overall, median urinary MBP levels were found to increase slightly from pre- to post shift, 44.8--48.8 ug/g creatinine (P=0.13) and median urinary MEP levels also increased from pre- to post shift, 70.1--76.1 ug/g creatinine (P=0.03). The results also confirm previous study findings that as a result of widespread usage, human exposure to both DEP and DBP are found to be significant, with DEP exposures occurring at higher levels than DBP. The overall findings of this study reaffirm that both dermal and inhalation exposures may contribute significantly to phthalate body burden. Future research should aim at determining specific sources of phthalate exposure and the proportional contribution of phthalates in commonly used massage products to total body burden.
