Self-rated health (SRH) may reflect both latent health and systematic error (reporting behaviour). To understand what SRH measures from the perspective of reporting behaviour I: (1) developed an operational definition of reporting behaviour and (2) assessed the degree to which reporting behaviour varied systematically by several health factors (age, gender, income, education, unemployment, smoking, drinking, exercise, diet, language and province). Considering the Health Utilities Index mark 3 as my measure of latent health, I regressed HUI on the five levels of SRH. The systematic variation that could not be explained by SRH was defined as reporting behaviour. This became the dependent variable in models used to assess whether reporting behaviours varied by systematically by the different health factors. Reporting behaviour varied systematically, statistically and clinically significantly, across age and socioeconomic status. There was strong optimism (positive reporting behaviour) among those 80+ years. Socioeconomic status was inversely associated with optimism.
