Young men who have sex with men (YMSM) are disproportionately affected by HIV/AIDS, yet we know relatively little about the impact of sexual identity development on sexual risk. This study utilized cross-sectional data to investigate pathways of HIV risk among a sample of young ethnically diverse sexual minority males (n=156) ranging in age from 14-24 years (x¯ =18.64). Path analysis was conducted to examine the predictive power of Fisher and Fisher's (1992) Information-Motivation-Behavioral skills (IMB) model compared to an extended model tailored for this population.

Results indicated that the extended model predicted substantially more primary (i.e., behaviors that can directly prevent HIV infection) and secondary sexual risk (i.e., behaviors that exacerbate one's HIV risk but do not directly prevent infection) for sexual minority male youth (r2 = .27 and .38) than the original IMB model (r2 = .13 and .08). The extended model fit the data well for both primary (chi2 = 34.63 [30], p=.26; RMSEA=.03; PGFI=.52; TLI=.97) and secondary sexual risk behaviors (chi2 = 45.23 [37], p=.17; RMSEA=.04; PGFI=.53; TLI=.95). Model fit was not entirely acceptable for the original IMB model for primary (chi2 = 3.70 [2], p=.16; RMSEA=.07; PGFI=.20; TLI=.94) or secondary risk (chi2 = 3.15 [2], p=.16; RMSEA=.06; PGFI=.20; TLI=.96). Path results for the extended model indicated that the primary sexual risk behaviors of young sexual minority males were predicted by (a) more negative attitudes toward LBGT people, (b) a sex-centered developmental sequence, (c) more frequent alcohol use, (d) less HIV prevention information, and (e) less HIV prevention motivation. Secondary sexual risk behaviors were predicted by two additional factors: (f) more frequent marijuana use and (g) non-male exclusive sexual orientation.

This study compared two conceptual models of HIV risk and identified some of the risk and protective factors associated with sexual risk for young sexual minority males. Most importantly, an extended model was developed that was more predictive of sexual risk than the original IMB model for sexual minority youth. These findings suggest that sexual identity development and substance may be important targets in HIV prevention with YMSM, in addition to traditional HIV-related cognitive and behavioral factors.
