Introduction: Studies on the relationship between treatment adherence and tobacco use reveal that smokers are less likely to follow physician recommendations than nonsmokers. Because asthma and chronic obstructive pulmonary disease (COPD) often require intensive and continuous daily self-management, it is necessary to determine what patient-level factors exist for smokers that influence their treatment decisions. The purpose of this study was to characterize the relationship between tobacco use and adherence in patients with asthma and/or COPD. Additionally, researchers sought to identify personal perceptions of smokers that may influence adherent behavior.

Objectives: The objectives of this study were to 1.) Determine if smokers with asthma and/or COPD are less likely to follow provider recommendations than nonsmokers, as well as, 2.) Probe differences in individual level perceptions (perceived susceptibility, benefits, and barriers) associated with adherence in these patients.

Methods: This study employed a cross-sectional design on a convenience sample of n=84 patients between the ages of 40 and 64 years old with a diagnosis of asthma and/or COPD. The study was conducted between November 2015 and February 2016 in the waiting rooms of the outpatient pulmonary clinics at the University Medical Center of New Orleans. Patients completed surveys while awaiting their clinic appointments. Bivariate and multiple regression analyses were performed using SAS 9.4. Smoking status, gender, race, and diagnosis were explored as predictors of adherence. Associations between individual level perceptions (perceived susceptibility, barriers, and benefits) and smoking status were also explored.

Results: Compared to nonsmokers, smokers were found to be less adherent to general and specific physician recommended measures of adherence including medication filling (p<0.0001), taking a medication for asthma/COPD (p=0.04), and having a yearly flu/pneumonia vaccine (p=0.003). Differences in patient level perceptions were also associated with smoking status as perceived susceptibility and perceived barriers were significantly higher in smokers (p=0.0023 and p<0.0001, respectively). Smokers also perceived fewer benefits to adherence than nonsmokers (p=0.0025).

Conclusion: Smoking was associated with lower levels of adherence across several measures. By identifying adherence-related perceptions held by smokers, targeted interventions may be developed that are tailored to the specific, yet often neglected, needs of this population.
