Background: Questions of resource allocation -- who gets what, and why -- have been present in public debate around medical spending for decades. But questions of the allocation of public health funds for different types of programs are present in everyday budget-setting, as well. With recent public health finance data, researchers are getting a sense of how much public health is spending; this project aims to fill a gap in our knowledge of why public health leaders make the priority-setting decisions they do.

Methods: Forty-five in-depth interviews were conducted with public health officials from 6 state public health agencies by telephone. Additionally, a complementary national survey regarding resource allocation practices and policies was fielded. We received 207 responses in the survey portion of this project (66% response rate). Approximately 7-8 positions were targeted in each agency in both the interview and survey stages.

Results: This dissertation characterizes the budget- and priority-setting processes at state health agencies. It identifies issues and factors that are critical to decision makers, whether these are shared across divisions, states, and individuals, as well as how various pressures (political, economic) affect the process. Additionally, the dissertation explores how different funding sources affect decision-making.

Conclusions: Implementation of the Affordable Care Act and changes recommended by the Institute of Medicine's recent report on public health funding suggest public health is again at a crossroads. At the same time, state public health leaders are being asked to make ever-more difficult decisions, as need continues to expand while funding contracts. This project aims to show how and why they make the decisions they do during times of scarcity.
