This dissertation, which addresses the fields of medical anthropology and science studies, explores the possibilities of defending and transforming a non-Western medico-social system in the face of a complex biomedical hegemony. This ethnography examines what becomes of Tibetan medical praxis---its unique disease etiology, its philosophy of health, and its complex pharmacology---when its formulas are made to perform according to the logic of biomedicine, and when commoditization is coupled with a de-valuation of practitioner expertise and a disaggregation of those who produce Tibetan therapies from those who prescribe them. Specifically, I analyze and critique: (1) The implementation of biomedically-derived Good Manufacturing Practices (GMP) on Tibetan medicine production; and (2) The application of Randomized Controlled Trial (RCT) methodologies and epistemological assumptions to the clinical trial of a Tibetan formula. Throughout this work, I present Tibetan medicine as a dynamic system that has been reformed, contested, and creatively re-imagined, both historically and against the backdrop of modern China's political economy, as well as regional and international investments in Complementary and Alternative Medicine (CAM). This thesis also explores implications of the commercialization of Tibetan medicine on Tibet's landscape and its people. As such, this dissertation illustrates some of the socio-economic, ethical, and epistemological challenges inherent in transforming so-called 'traditional medical systems' for mass markets, as well as possibilities for creative turns and unexpected outcomes through this metamorphosis. Through the lens of Tibetan medicine, this study also critically engages a key biomedical concept: efficacy. I consider how efficacy is defined, regulated and used in the interstices of western biomedicine and its Others. Choosing efficacy as the key object of my analysis has also enabled me to illustrate the various levels of what I call a 'double standard' that exists between biomedicine and non-biomedical praxes. Finally, in this work I propose an 'anthropology of efficacy', in which efficacy can serve not only as an object of critique, but also as an organizing principle of ethnographic research, analysis, and engaged, multi-disciplinary social science.
