Problem: The prevalence rate of preschool children is approximately 9% (CDC, 2017). Limited evidence exists regarding the effectiveness of an intervention that focuses on nutrition and activity education, and considers the influence of caregiver self-efficacy in the management of overweight and obese preschool children in the primary care setting. The few studies that have measured the effects of intervention programs involved integration within a school or tertiary medical setting. Furthermore, much of the body of research focuses on the overweight and obese school-age child and adolescent rather than the preschool child. Method: This two group, quasi-experimental study measured the effect of a nurse practitioner led healthy weight intervention on body mass index, nutrition, physical activity, and sedentary activity behaviors in the preschool child, and caregiver self-efficacy levels. Child-caregiver dyads in the control group received educational information appropriate to the development pertaining to the age of the child. Child-caregiver dyads in the experimental group received an intervention that discussed the importance of adhering to: (1) consuming five fruits and vegetables daily, (2) two hours of screen time viewing daily, (3) one hour of physical activity daily, and (4) zero consumption of sugary sweetened beverages. Follow up visits occurred at one, two, and three months after the initial visit for the experimental group, and three months after the initial visit for the control group.Results: There were no statistically significant differences between the control group and experimental group after participating in the nurse practitioner led healthy weight intervention.Conclusions: The improvements in the experimental group's body mass index, fruit juice consumption, fruit and vegetable consumption, physical activity levels, and sedentary activity behaviors indicate that the educational intervention is promising, however statistical significance was not met. Future studies must explore other behaviors associated with the incidence of overweight and obesity in preschool children, and determine the barriers to self-efficacy that influence the nutrition and physical activity levels in preschool children. Additionally, future studies must consider measuring behaviors using objective tools, and monitoring body mass index, and nutrition and physical activity behaviors over a one year period.
