This study of hospitalized acute myocardial infarction (AMI) patients tested whether patient judgment relates to quality of technical care (QTC). QTC is defined as whether actual treatment matched the AMI criteria of American College of Cardiology and the American Heart Association (ACC/AHA). Using a criteria-mapping approach, I and several cardiologists developed a model to evaluate the care provided to see how well it matched ACC/AHA standards. The model was validated by comparison with holistic judgments of cardiologists about the proper care. The patients' judgment was significantly related their perceived quality of life and physical function. A weak relationship appeared between their judgment of pain management, and of the skill of the intravenous starter, with the appropriateness of treatment. However, on the most important question, I found no relationship between the overall quality of technical care and patient judgment about the quality of care, as measured on a post-discharge survey. Furthermore, patient judgment was not related to age, gender, education level, income, marital status, or which of the four medical centers the patients were treated in. Thus patient judgment is not a proper, single indicator for evaluating the quality of technical care. Patient may make their judgments about quality of care based on factors that are not relevant to actual quality of technical care.
